Just a suggestion that kind of contradicts my earlier comment that supports labelling officially named locations and things, but I saw this on the Avatar Wiki and thought that this could partially solve the problem. They have a small notification box at the top of the page below the title that says quote:
"Well, that's not its name ... but ...
The term "[INSTERT PAGE NAME]" has not been confirmed by official sources."
See Eastern Sea on their wiki and you'll see what I mean. Maybe something like this could be used instead of another one of many icons at the top of the page. This makes the point easily seen. Just a suggestion.
While I like this suggestion, I think that the box would be too big, and it would be an issue mainly for pages that already have a similar box (like Prince Charming, who has a box with "This article focuses on Prince Charming's Enchanted Forest persona. For his Storybrooke counterpart, see David Nolan. For the character he impersonated, see Prince James.")
Twp good options I saw on the Kirby Wiki:
1 - Adding a template next to the page's name which would be an asterisk (*), and when you hover the mouse over it it would say "This article's name has never been confirmed by official sources so it has been given a fitting name." or something like that. (see this page for an example, see the asterisk (*) next to Big Dice on the list, it says "Conjectural name".)
2 - Almost the same thing Nick said, but the box is slightly different (flatter but longer, see this page for an example)
